１ 調達内容  品目分類番号 22、31  調達件名及び数量 医療用画像管理システム 一式  調達案件の仕様等 入札説明書及び仕様書 による。  履行期間 契約締結日から令和９年３月31 日  履行場所 国立療養所邑久光明園  入札方法 落札決定に当たっては、入札書 に記載された金額に当該金額の10パーセント に相当する額を加算した金額（当該金額に１ 円未満の端数があるときは、その端数金額を ている者。  次の各号に掲げる制度が適用される者に あっては、この入札の入札書提出期限の直近 ２年間（及びについては２保険年度）の 保険料について滞納がないこと。 厚生年金保険 健康保険（全国健康保険 協会が管掌するもの） 船員保険 国民 年金 労働者災害補償保険 雇用保険  入札時において厚生労働省から指名停止の 措置を受けている期間中の者でないこと。  入札に参加を希望する者は、暴力団等に該 当しない旨の誓約書を提出しなければならな い。  その他予算決算及び会計令第73条の規定に 基づき、支出負担行為担当官が定める資格を 有する者であること。  この入札の入札書提出期限の直近１年間に おいて、厚生労働省が所管する法令に違反し たことにより送検され、行政処分を受け、又  入札保証金及び契約保証金 免除。  入札者に要求される事項 この一般競争に 参加を希望する者は、１のに示した調達内 容を履行できることを証明する書類と２の の資格審査結果通知書のコピーを添付して、 入札書の受領期限までに提出しなければなら ない。 また、入札に参加を希望する者は、入札書 の提出時に、支出負担行為担当官が別に指定 する暴力団等に該当しない旨の誓約書等を提 出しなければならない。 入札者は、開札日の前日までの間において、 支出負担行為担当官から当該書類に関し説明 を求められた場合は、それに応じなければな らない。  入札の無効 本公告に示した競争参加資格 のない者の提出した入札及び入札に関する条 件に違反した者の提出した入札書は無効とす る。 また、入札に参加した者が、の誓約書を 提出せず、又は虚偽の誓約をし、若しくは誓 約に反することとなったときは、当該者の入 札を無効とするものとする。   ) 号    第 達 調 府 政 外 号 ( 報 官 日 曜 火 日  月  年  和 令  契約書作成の要否 要  落札者の決定方法 予算決算及び会計令第 79条の規定に基づいて作成された予定価格の 制限の範囲内で最低価格をもって有効な入札 を行った入札者を落札者とする。  その他 詳細は入札説明書による。 ５ Summary  Official in charge of disbursement of the procuring entity : Sohma Daisuke, Director, Administration Department, National Sa‑ natorium Okukomyoen  Classification of the services to be pro‑ cured : 22, 31  Nature and quantity of the products to be purchased : Picture Archiving and Commu‑ nication System, 1 set  Delivery period : From the date of con‑ tract signing until March 31, 2027  Delivery place : National Sanatorium Okukomyoen  Qualification for participating in the ten‑ dering procedures : Suppliers eligible for participating in the proposed tender are those who shall :  not come under Article 70 of the Cabi‑ net Order concerning the Budget, Audit‑ ing and Accounting. Furthermore, mi‑ nors, Person under Conservatorship or Person under Assistance that obtained the consent necessary for concluding a contract may be applicable under cases of special reasons within the said clause ;  not come under Article 71 of the Cabi‑ net Order concerning the Budget, Audit‑ ing and Accounting ;  have Grade A, B or C Sales of prod‑ ucts for participating in tenders by Min‑ istry of Health, Labour and Welfare (Single qualification for every ministry and agency) in Chugoku Region in the fis‑ cal years 2025, 2026, and 2027 ;  The following persons are ineligible to participate in the competition : ａ Those who have included false infor‑ mation in their qualification applica‑ tion or supporting documents. ｂ Those whose business situation or creditworthiness has deteriorated dras‑ tically.  there not being nonpayment about the premium for most recent two years in a deadline for bid book presentation of this bid about the person whom a system to advocate next is applied to ; ａ welfare annunity insurance ｂ health insurance ｃ seamens insurance ｄ national pension ｅ workers compensation insurance ｆ unemployment insurance  not be suspended from transactions by the request of the officials in charge of contract.  Those wishing to participate in the bid‑ ding must submit a written pledge stating that they are not affiliated with orga‑ nized crime groups or similar organiza‑ tions.  meet the qualification requirements which the Obligating Officer may specify in accordance with Article 73 of the Cabi‑ net Order.  In the year immediately preceding the deadline for submitting bids for this tend‑ er, any person who has been referred to the public prosecutors office, received administrative sanctions, or received ad‑ ministrative guidance (limited to those made public by administrative agencies) for violating laws and regulations under the jurisdiction of the Ministry of Health, Labour and Welfare will not be likely to jeopardize the fair implementation of this project or the securing of public trust in this project.  Those wishing to participate in the co‑ mpetition must submit a Self‑Declara‑ tion Form by 12 : 00 PM on October 22, 2026.  Time‑limit for tender : 12 : 00 P.M.
